What is an Intensive Outpatient Program?
An Intensive Outpatient Program (IOP) provides structured addiction treatment at a moderate intensity level, allowing clients to live at home and maintain daily responsibilities while receiving clinical care. IOP sits below PHP and above standard outpatient on the continuum of care.
At Ascend, IOP offers both 3-day and 5-day weekly schedules. Sessions are structured around evidence-based therapies and are available during daytime and evening hours. Virtual attendance is available for clients who qualify, extending access across the state of Florida.
Who is IOP appropriate for?
IOP is designed for individuals who need consistent clinical support but can function safely in their daily environment. Common candidates include:
- Clients stepping down from PHP who are ready for reduced treatment intensity
- Individuals with mild-to-moderate substance use disorders who do not require daily programming
- People returning to work or school who need treatment that fits their schedule
- Clients with a stable home environment or who are living in a recovery residence
- Individuals re-engaging in treatment after a relapse who do not need residential care
The clinical team determines whether IOP is appropriate through a comprehensive assessment. Clients can step up to PHP if more intensity is needed or step down to standard outpatient as they progress.

What does the IOP schedule look like?
Ascend offers two IOP scheduling tracks:
- 5-Day IOP: Monday through Friday, 5 hours per day (25 hours weekly). This track mirrors PHP intensity and is appropriate for clients who need maximum outpatient structure while living off-site.
- 3-Day IOP: Three sessions per week, 3 hours per session (9 hours weekly). This track accommodates work and school schedules. Evening sessions are available.
Both tracks include group therapy, individual therapy, and psychiatric services. The clinical team may recommend transitioning between tracks as the client progresses. Virtual sessions are available for qualifying clients, allowing participation from anywhere in Florida.
“IOP exists to meet people where they are. Not everyone can put their life on hold for treatment. The clinical outcomes we see in IOP confirm that structured, evidence-based outpatient care produces real, measurable recovery.”
What therapies are included in IOP?
IOP at Ascend incorporates multiple evidence-based modalities tailored to each client's treatment plan:
- Cognitive Behavioral Therapy (CBT): Identifies and restructures thought patterns that drive substance use
- Dialectical Behavior Therapy (DBT): Builds distress tolerance, emotional regulation, and interpersonal effectiveness skills
- Family therapy: Addresses family dynamics, communication patterns, and the role of the family system in recovery
- Relapse prevention: Skills-based groups focused on trigger identification, coping strategies, and recovery planning
- Holistic modalities: Mindfulness, yoga, art therapy, and life skills training as clinically appropriate
- Psychiatric consultations: Medication evaluation and management for co-occurring mental health conditions
How long does IOP last?
IOP at Ascend typically lasts 6 to 12 weeks. Duration is determined by clinical progress, treatment plan goals, and insurance authorization. The clinical team conducts weekly reviews and adjusts the plan as the client progresses.
Upon completing IOP, most clients transition to standard outpatient care for continued support. Research consistently shows that longer engagement in structured treatment leads to better long-term outcomes. Clients who complete both PHP and IOP report significantly higher rates of sustained sobriety at one year.
